INCIDENT REPORT
January 2002

On January 11,2002 the filter press failed to start. The start-up sequence was reviewed but after several attempts the press would not engage. The operator noted that the hydraulic motor would not start and did not appear to have power. JWI Technical support was contacted by telephone to assist in troubleshooting the problem. Certain basic observations were made and it was determined that the problem was electrical. The Operator made arrangements with Britt Luther to come to site on Monday 1/14/02 to assist with electrical measurements.

1/14/02: Britt Luther was on-site to troubleshoot the system. Several electrical measurements were made with the specific problem traced back to the filter press electrical panel. JWI was contacted once certain measurements were made and it was determined that the problem was most likely the motor starter. A replacement part was ordered from Kirby Risk and arrangements were made with Anbro to have electrician install the part.

1/15/02: The motor starter was purchased and Anbro Electric contacted to install it.

1/16/02: The motor starter was installed, which solved the power problem with the hydraulic motor, but the pumps would not activate to close the press. The hydraulic system was checked and the hydraulic fluid level was found to be low.

1/17/02: Hydraulic fluid was purchased and the reservoir was filled to the proper level. The filter press was brought into working order. Note: The filler cap was found to be loose on the reservoir. This may have led to some loss of the fluid. No leaks were evident.
