Dear Ms. Fliss;

As per Supplement to Master Agreement For hazardous Substance Or Petroleum Response Services between the Indiana Department of Environmental Management and Earth Tech, please find the Monthly Report of Operations of the Illinois Central Spring Treatment Plant, Bloomington Indiana for the reporting period of December 2001.

Operational Status - The beginning of the month started with the SRS Tanks full and overflowing for a part of one day. Within four days of the latest rainfall the tanks were drained and prepared for the next event(s).

An extended period of wet weather occurred during the middle of month with two storm events accounting for more than 3.5 inches rainfall over a six-day period. The SRS Tanks were able to contain the first part of the series but the system began an overflow condition for the following 4 days. The plant had operated in a normal mode throughout the initial storm event but on December 18 (extended flow) the GAC units were recording pressures that indicated that a backwash was needed. The system was reduced to a 200-gpm process rate and the lead GAC units were backwashed on a shortened cycle. This backwash cycle temporarily placed the plant back into 100% operation but within the day the head pressures had increased where a complete backwash cycle would be required. The IDEM Project Manager was contacted by the operator and the system was placed on a 200 8pm processing rate while the Thickener and the GAC units were prepped and backwashed. See the attached incident report for a more detailed explanation.

The maximum rainfall during a 24-hour period was 1.60 inches with a total of 3.92 inches recorded for the month. A total of 9,555,735 gallons of water was diverted to storage with 3,092,690 gallons returned for processing and 6,463,045 gallons of water passively treated through the SRS Tanks as an overflow during these storm events. At the end of the month the SRS Tanks were not holding water. No bypass of active or passive treatment systems occurred during this month.

Corrective Measure Taken or Planned - None to report for December 2001.

Analytical Results - Analytical results for the reporting period are attached to this report on the Event Sampling Analysis Log.

Unexpected Maintenance Requirements - There were no unexpected maintenance requirements reported during this period.

Evidence Of Trespassing - There was no evidence of trespassing at the facility during the

reporting period.

Report from Visitors Log - The Visitor's Log is available for inspection at the ICS office. Copies of this log are available to authorized personnel upon request and authorization of the IDEM Project Manager. There were no telephone calls requesting tours from the general public but there was one request by Tetra Tech (EPA Remediation Contractor), Raj Arar and Andy Suminski to tour and review the plant.

Summary of Costs - Total costs for this Project for the reporting period of December 2001 (through 12/2101 ) was $21,845.73. The summary of cumulative project costs and comparisons to the project budget are attached to this report.
